Add New Certificate/Degree Form   

Description: Welcome to the Outcomes page. This page displays the Add Certificate/Degree form. This form allows users the capability to Add a New certificate/Degree record to the participant's case.

Add New Certificate/Degree Record

Complete the following to Add a New Certificate/Degree record:

  • Fill out all required fields on this form.

    • In the Date Entered Program Education text box, enter the date the participant entered the educational program. Users can also use the image111.gif (Calendar) icon to select the date. (Required Field)

    • In the Attained Diploma drop-down list, select the diploma attained by the participant. (Required Field)

    • In the Date Attained Diploma text box, enter the date the participant attained his/her diploma. Users can also use the image111.gif (Calendar) icon to select the date. (Required Field)

    • In the Name of Diploma text box, enter the name of diploma attained by the participant.

  • Once all required fields have been completed, click the image112.gif (Save) button. This will display the Successfully Saved Message.

  • Click the 'here' link to return to the Outcomes page. The newly created Certificate/Degree record will display under the Outcomes Overview section.
