Voluntary Repayment of Advances (Specific)
This page is a letter template for requesting a specific repayment of
advances.
The addressee information (the Secretary of Labor name, the Office of
Workforce Security Administrator, and address) is dynamically populated
by the system.
All fields are required.
Enter the date of request (mmddyyyy format) in
the date field at the top of the letter.
In the first field of the body of the letter,
enter the date the State was approved for making voluntary repayment requests
(mmddyyyy format).
In the Date field, enter the date (mmddyyyy format)
the funds for repayment are to be transferred.
In the Amount field, enter the amount to be repaid.
Enter whole dollars only; the range of values for the dollar amount is
1 to 999,999,999.
Next, indicate contact information in one of three
ways:
Check the box "me at (Signatory Official
telephone number pre-populated based on the information in the system)",
or
Check the box for a contact other than the
Signatory Official. If checking this box, enter the name, title, phone
number, and extension (if applicable) of the point of contact, or
Check both boxes to list both the Signatory
Official and another contact. Enter the name, title, phone number, and
extension (if applicable) of the other point of contact.
The Signatory Name and Title are also dynamically populated by the system
based on the Signatory Official on file with the overseeing office at the DOL. If the information needs to be corrected, please contact the U.S. Department of Labor.
The Personal Identification Number (PIN) is not required to save a draft
of the Request for Advances, but is required in order to submit the request
to the overseeing office at the DOL.
Below the field to enter your PIN there are 4 buttons.
Save Draft:
click this button to save the information entered in this letter without
submitting it to DOL OWS.
Submit to DOL:
click this button to submit this letter to DOL OWS.
Reset:
click this button to clear all information entered in the fields for this
letter and restore the default or saved values.
Cancel:
click this button to leave the letter without saving and return to the
previous screen.
After a request for advances has been submitted to DOL, the letter template
displays as read-only.
